This Privacy Policy explains how Alleviate Care ("Alleviate," "we," "us") collects, uses, and shares information when you visit alleviate.care (the "Site"). It applies to the marketing website only. The Site is not used to collect patient health information. How Alleviate handles protected health information as part of care is described in our HIPAA Notice of Privacy Practices, not this policy.
Information you give us. When you use a contact form, we collect the details you enter, such as your name, your role, your organization, your phone number or email, and the content of your message. Please do not enter patient names, diagnoses, or other health information in these forms; clinical referrals are handled by phone, fax, or your EMR, never through the Site.
Information collected automatically. As with most websites, our hosting and security providers log technical data when you visit, such as your IP address, browser type, the pages you request, and the date and time. We use this to operate, secure, and troubleshoot the Site.
Security and anti-fraud. Our forms use Cloudflare Turnstile to tell people apart from automated bots. This is a strictly necessary security measure.
Cookies and analytics. The Site uses only strictly necessary cookies by default. If optional analytics are offered, they load only after you opt in through our cookie banner, and you can change your choice at any time. We do not use cookies for cross-context behavioral advertising.
We do not sell your personal information, and we do not share it for cross-context behavioral advertising.
We share information only with service providers who help us run the Site and our communications, under contracts that limit how they may use it, for example, Microsoft 365 to deliver and store form submissions (covered by a business associate agreement) and Cloudflare for hosting and security. We may also disclose information where required by law or to protect the rights, safety, and security of our patients, staff, and organization. We do not sell personal information.
We keep contact submissions for as long as needed to respond and to maintain reasonable business and compliance records, after which we delete or de-identify them. Technical logs are retained for a short period for security and troubleshooting.
Information is transmitted over encrypted connections (HTTPS), and access is limited to authorized personnel. Form submissions are delivered into a business-associate-covered Microsoft 365 environment, and the Site is designed not to collect health information.

Alleviate operates a platform of community-based hospice providers in California. In order to better serve its patients and to help educate individuals and the general public about health care and medical issues, Alleviate owns and operates the Alleviate Web sites, including but not limited to: www.alleviate.care, (collectively, “Alleviate Web sites”).
Alleviate provides online educational and tools which are described below. (These are called “Services” below.) Alleviate has published these Terms of Use and Privacy Policy to inform Users of their rights and responsibilities in using the Alleviate Web sites.
Please note that these Terms of Use and linked policies only apply to the Alleviate Web sites. Alleviate provides links to third-party websites that are not operated by Alleviate. Alleviate users who link to a third-party Web site must recognize that our Terms of Use and other policies do not extend beyond the borders of the Alleviate Web sites. You may also download information from the Alleviate Web sites to your personal computer or other devices. If someone else uses your personal computer or other devices, they may be able to use these functions to gain access to your account or personal information. Alleviate cannot prevent or be responsible for protecting your personal computer or other devices against unauthorized use. You are solely responsible for protecting the personal computers or other devices you use against unauthorized use to access your account.
If you use a personal computer or other device provided by or shared with your employer, parent, spouse, roommate or other person, that other party may have the legal right to view all information stored on it. Alleviate cannot prevent or be responsible for such activities. If you use a personal computer or other device that another person has the legal right to control, you access your Alleviate account at your own risk.
Whenever you are done using the Alleviate Web sites you should log off the Web site, and you should not leave your computer or other device unattended when it is logged on to the Alleviate Web sites and someone else may be able to view the screen or use the computer or other device. Affordable antivirus and antispyware software is readily available for personal computers. Unauthorized individuals (“hackers”) and malicious software can sometimes gain access to computers through Internet connections, unless the computer has a working firewall. Jurisdiction and venue for any dispute arising from or pertaining to these Terms of Use shall be in the state of Califorinia.
If any provision of these Terms of Use is found to be invalid by any court having competent jurisdiction, the invalidity of such provision shall not affect the validity of the remaining provisions of these Terms of Use, which shall remain in full force and effect. No waiver of any of these Terms of Use shall be deemed a further or continuing waiver of such term or any other term.
